How often do you allow users to change their names?

When you run a forum, you don't want to let users change their username often. Sometimes allowing a user to change their name could mean people will forget how they acted in the past. For example, if someone was caught saying offensive things on the forum and was banned once, but then gets a name change, it would almost be like a they are trying to hide the bad stuff they did. I think users should keep their usernames forever. What do you think?

If it's an offensive name, that's an obvious name change, and I allow a user one free name change if the name they originally chose is offensive. But other than that, names stay put.

I have not put any restrictions on users about changing their usernames, users can change their usernames at their wish. Forums are communities where people can relax, learn, and share, I do not like to put any restrictions on users.
