Adsense, Media.Net, and Infolinks are some of the big ad networks for publishes. They have strict approval criteria, they have a high withdrawal threshold. Because of the requirements, a lot of people cannot use big ad networks, so they end up using smaller ad networks. The ad networks have low approval criteria, and low withdrawal thresholds, thus making small publishers' favorite ad network

People could manually place advertisement and the rotation script is available everywhere but this method seems as classic one and you will not get orders like those you mentioned you start earning from first day, people prefer A Ads but they have low income payment schedule.
