How do you manage trolls?

Say you have a few members that just make a mess of things and always break the rules and make other members uncomfortable or angry. You may decide to ban them because of this but they continue to rejoin and cause issues under various IPs and e mails. How do you manage this?

I have had spam account issues, I once was getting over 100 spam posts every day on my forum, however, I never have had any issues with trolls. If there are trolls on my site, I will simply ban his account. If he comes with another account, and starts creating a mess, I will ban the account again.
